Admission Requirements
Students must have a current application on file and all registration fees paid to be eligible for admission. The following documents must be included with the application:
- A certified copy of the child’s birth certificate
- Current immunization records.
- (Students 5 years old and above must have a Compliance Form issued by the Health Department)
- Social Security Card
Accident Insurance
One of the following is required
- Student Accident Form (Available through the office)
- A copy of current insurance coverage maintained by the family, certifying insurance covering personal injury of the student.
